Is the blood just running from the top of the shell down or bleeding from the bottom too. Your gonna have to dry dock it and make sure the bleeding stops. Apply anticeptic ointment to help heal the area. Jut treat like would if you got a bad cut. Personally myself i even used a bandaid on the shell. It will stay on while the turtle is dry docked. Once per day soak him and feed and then go straight back to dry docking till the area looks better.

You should look for a vet in you area, if that much is soft and you said the turtle is losing weight. Sounds like it has Metabolic Bone Disease.

Just make sure you got cuttlebone, feeding it plenty of pellets. Id even be generous with the amount, and make sure you have a bulb that produces uvb to help absorb calcium. Its really the only thing you can do. Where do you live so we can help locate the nearest herp. vet.

If it is metabolic bone disease, diamondbacks4life is correct, without a vet, all you can do is provide calcium and UVB. Both are needed.

Here's the home page for the Biology dept at the University of Tirana

http://biologjia.fshn.edu.al/

I'm betting someone there can either look at your turtle or recommend someone.

And here's a Slovakian herpetologist who did filed work in Albania in 2007 and 2013. The page lists his email address. He may know someone in Tirana

If there is still bleeding, I would try to keep the area clean and dry.
