Well, letting him eat only fish is not a good thing. Since the water/basking temps seem OK, it's impossible for us to diagnose what the problem may be. Lethargy is never a good sign, which is why you need to take him to a vet.

One thing he does a lot is his head is always laying down, also when I put him in the water he will eat just fish I think he hates everything else.

You've had them for 3 weeks? If so, it's possible that one or both were sick when you got them. That said, you need to find out what's wrong with your turtle and only a herp vet can determine what's wrong. Don't delay--the sooner you have your turtle seen, the better his chances of a speedy recovery. And please stop the fish. His diet sounds really unbalanced, and the fish could be contributing to his illness. Let us know what the herp vet says.

The fish might have a disease o.o.
