yes i use a repti basking spot lamp (75w) UVA rays... "the ultimate basking light for all types of reptiles"? which i bought from petco, i've gone through two.

he eats pellets for turtles & shrimp treats. sometimes mealworms, but there are feeder fish in the tank which they are not eating.

That's fine for a heat lamp (but you could use a regular light bulb for that), but what about the UVB light (you don't have one if the spot lamp is all you're using; see what I said about UVB lights in my previous post). Your turtle isn't getting UVB rays, which are converted to Vit D3, which, in turn, is needed for him to absorb the calcium in his diet... Get him a UVB light IN ADDITION to the light he has now.
His diet contains too much phosphorus, which also will work against calcium absorption.

No wonder the poor thing's shell is soft.

He needs a change in habitat (UVB light) and a change in his diet.
